This form is to be used for Off-Campus Trip in place of the SEIF.

The form must be completed by the organization president, treasurer or an executive board member of the undergraduate student government e-board member.

The Office of Student Activities will not review or approve any trips without all information and required documentation.

Do not attempt to submit this form without all required documentation.

The individual submitting this form:

1- Takes full responsibility for the event/trip and acknowledges that it has been authorized/ approved by the sponsoring group.

2- Indicates that all information provided is correct and all-inclusive.

3- Indicates that copies of all contracts, agreements, etc. for this trip have been affixed to this form.

4- Acknowledges that the filing of false information will result in the immediate cancellation of the trip.

5- Acknowledges that the club/organization may not proceed with the event until the OSA/Student Affairs representative approves the trip.

Due to COVID-19 reopening plan guidelines, the off-campus trip safety plan must be attached to the form.
