Referral Form

As a faculty member, you can refer a student to the RWC by completing the form below and asking the student to bring this form with her/him when registering at the Center.The information you give us will help us to structure our sessions with your student. Bear in mind that we cannot guarantee an appointment to every student, so it's a good idea to send your student as early in the semester as possible. In order to help us assess your student's needs, we would appreciate if s/he could bring a writing sample. You will receive a progress report at the end of the semester reviewing the student's work at the Center.

Please note: submitting this form to the RWC does not inform a student that he/she has been referred, nor will the RWC contact your a student on your behalf. Therefore, you must either forward your receipt to your student or print it out and give it to him/her.
