Enter the email address of the primary contact person for this unit. This is the email address that will be used to notify the primary contact person when the new website is created.

Use ONLY lowercase letters and dashes. Spaces are not permitted. e.g. "School of Education" would become "school-of-education" The WEBSITENAME entered here should also be used for the contact email address below, if you do not already have a dedicated email address.
This email address will be displayed at the bottom of each page on your website. If you already have an email address you'd like to use for this purpose, enter it here. Otherwise, enter the same WEBSITENAME you entered above in the Requested Subsite Address field, and a new email address will be created for you.
This phone number will be displayed at the bottom of each page on your website. To omit the phone number, all fields must be empty.
This physical address (office room number and building) will be displayed at the bottom of each page on your website. To omit the location, leave this field blank.
Enter the name and email address of the person who will be responsible for managing the website and deciding "who will do what" within the site.
Enter the full names and email addresses of any other people in your unit who may be working on the website. These people will be given access to the CMS so that your Staff Manager can assign them roles.
